While  most  academic  surgeons  recognize  Dr.  Moody,  as  a  top  echelon  surgical  leader  of  the  last  half  century,  we  choose  to  dedicate  this  edition  to  him  because  of  the  profound  influence  he  had  on  the  careers  of  many  of  the  editors  of  this  textbook.  To  some  of  us,  Dr  Moody  was  our  surgical  chair  and  academic  inspiration.  To  others  he  was  a  research  collaborator.
