AB One  morning  in  2013,  Ted  Carroll  (not  his  real  name),  a  man  in  his  mid-sixties,  was  awakened  from  a  sound  sleep  by  a  sharp,  burning  pain  in  his  chest  that  radiated  down  his  left  arm.
