PY - 2022 T2 - Current Medical Diagnosis & Treatment 2022 AB - Key Clinical Updates in Hypertrophic CardiomyopathyThe following classes detail the 2020 AHA/ACC guidelines for a preventive ICD:– Class I: Patients with documented cardiac arrest or sustained ventricular tachycardia.– Class IIa: (1) sudden death in ≥ 1 first-degree or close relative ≤ 50 years of age, (2) any LV wall ≥ 30 mm, (3) any recent syncope likely to have been arrhythmogenic, (4) LV apical aneurysm, or (5) LV systolic dysfunction (EF 15%) late gadolinium enhancement on cardiac MRI. In those who receive an ICD, antitachycardia pacing should be programmed to minimize shocks.The use of an ICD is contraindicated, though, if the purpose is simply to allow for the patient to play competitive sports.
