Most authorities agree that nutritional support is indicated for at least four groups of adult patients: (1) those with inadequate bowel syndromes, (2) those with severe prolonged hypercatabolic states (eg, due to extensive burns, trauma, mechanical ventilation), (3) those requiring prolonged therapeutic bowel rest, and (4) those with severe protein–calorie undernutrition with a treatable disease who have sustained a loss of 10% body weight at 6 months or 20% body weight at 1 year. In most other conditions, it has been difficult to prove the efficacy of nutritional support over treatment without such support.
