This collapse is related to the relatively long length and narrow diameter of the right middle lobe bronchus and the oval (“fish mouth”) opening to the lobe, in the setting of impaired collateral ventilation. Fiberoptic bronchoscopy or CT scan is often necessary to rule out obstructing tumor. Foreign body or other benign causes are common.
